I have artheritis in my hands and use pliers to tighten the nut enough to hold the mouthpiece in correct position. If the entire nut were made of metal, rather than the plastic outer portion, it would not crack and break. Most people using a cpap are older with less strength in their hands and fingers needing pliers to position the mouthpiece
